RN Hospice Admissions

This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Position

Responsibilities

• · Coordinate and oversee all aspects of new patient admissions
• · Collaborate with patient care teams to support seamless communication and continuity of care
• · Educate patients and families regarding condition, needs, and care
• · Perform head-to-toe physical assessments
• · Complete all required documentation accurately and on time

Requirements

• · Current Registered Nurse license
• · Current driver's license
• · Current CPR certification
• · Two years of Registered Nurse experience in a clinical care setting
• · One year of full-time hospice experience
• · Reliable transportation and valid automobile liability insurance
