AdvisaCare is seeking compassionate and skilled PRN (as needed) Home Health Registered Nurses to provide exceptional nursing care in patients' homes. Key Responsibilities

• Provide skilled nursing care and assessments to patients in their homes on an as-needed basis.
• Develop and implement individualized care plans according to physician orders.
• Collaborate with interdisciplinary teams and communicate effectively with patients and their families.
• Maintain accurate and timely documentation of patient care.
• Ensure compliance with health, safety, and regulatory standards.

Requirements

• Current Michigan Registered Nurse (RN) License.
• Minimum one year of home health nursing experience preferred.
• Valid driver's license and dependable transportation.
• Excellent clinical assessment, organizational, and communication skills.
• Ability to work independently and adapt to varying patient needs.
• CPR certification required.
• Must pass background check and drug screening.
