About this role

11,000 Americans turn 65 every day. Help build how they get care, right where they live.

AllCare.ai grew to 2,500 seniors in 150 senior living communities across California in under two years, running on our own AI-powered care coordination platform. We're expanding to the Philadelphia suburbs and hiring nurse practitioners to lead care for residents of local personal care homes and assisted living residences.

What you'll do

  • Complete initiating visits and provide proactive primary care in senior living communities
  • Enroll residents in our care management programs (CCM, APCM and PCM) and build their care plans with our coordination team
  • Work clustered routes, with telehealth where allowed
  • What's in it for you
  • $45 per patient visit, about $540–$675 on a typical 12–15 patient day
  • AI-powered charting, documentation and routing support, so your time goes to patients
  • A physician lead and a care coordination team behind you
  • Full-time or part-time, W-2 or 1099

Requirements

  • You're a fit if you
  • Hold an active Pennsylvania CRNP certification with prescriptive authority and national board certification (FNP or AGNP preferred)
  • Are enrolled in Medicare or able to enroll, and have (or can get) a Pennsylvania DEA registration
  • Have geriatric, primary care or long-term care experience (preferred)

Benefits

You'll practice under a collaborative agreement with our partner physician, as Pennsylvania requires.
