What You'll Do
- As a Weekend Start of Care RN, you'll play an important role in helping patients begin their home health journey.
- You'll:
- Complete Start of Care (SOC) assessments and OASIS documentation
- Evaluate patients and develop individualized plans of care
- Coordinate care with physicians and the interdisciplinary team
- Ensure patients receive a smooth and successful transition to home health services
- Provide education and support to patients and their families
- Help patients achieve their healthcare goals and remain safely at home
What We're Looking For
- Current, active Pennsylvania RN License
- Previous Home Health experience REQUIRED
- Strong OASIS and Start of Care assessment experience
- Excellent clinical judgment and assessment skills
- Independent, dependable, and organized
- Valid driver's license and reliable transportation
