Position Summary
- Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.
Education
- Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.
Licenses and Certification
- Illinois license in Physical Therapy

- Technical Experience: N/A
Role Specific Responsibilities
- Performs thorough evaluation
- Develops appropriate Plan of Care
- Implements Plan of Care
- Documentation is accurate, complete and timely
- Provides supervision to assistants, rehab techs, and students
