About this role
Title: Lead Physical Therapist
- Pelvic Floor Program
Reporting to: Head of Clinical Operations
Position Class: Lead Clinician / Program Lead
Location: Remote (U.S.-based)
About the Role
LainaHealth is a national virtual medical practice that pairs licensed clinicians with a web-based AI assistant, Laina, to make care more accessible and affordable. We deliver virtual physical therapy for health plans, health systems, and provider networks across the country under an integrated, medical model and consistently achieve 3X the engagement at 1/4 of the cost of traditional care without compromising outcomes or satisfaction.
We are looking for a Lead Physical Therapist to establish, treat within, and build our Pelvic Floor Program. This is a clinician-first role - you will treat patients from day one, build your pelvic floor caseload as referrals arrive, and be the clinical authority for how pelvic health is delivered at LainaHealth..
What makes it distinct from a staff PT role is that you also own the program: defining the care model, building the exercise library and protocols, selecting outcome measures, and setting the standard that future pelvic floor clinicians will be hired and trained against.
Initial focus will center on a health system partner that is ready to refer pelvic floor patients to a white-labeled, multi-state virtual program. The conditions in scope span the full pelvic health spectrum - pregnancy and postpartum, urinary and bowel dysfunction, pelvic pain, pelvic organ prolapse, sexual dysfunction, and perimenopause and menopause - across patients of all genders. We expect referral volume to build gradually.
Because referrals will ramp up slowly, the role has two parts. Early on, much of your caseload will be general orthopedic and musculoskeletal patients, LainaHealth's core practice, while you build the pelvic floor program alongside your clinical work. As pelvic referrals grow, your caseload will shift toward pelvic floor, with orthopedic patients filling remaining capacity, until pelvic floor becomes the majority.
We expect you to be comfortable moving between the two and to build the program in step with your clinical work. Sized as a strong individual contributor with program leadership layered in. As pelvic floor volume grows, there is a clear path to hire, train, and lead a team of pelvic health clinicians around the program you built.
This role requires an AI-integrated workflow. We are an AI-native company and we expect every function to use AI tools deliberately across their day - documentation, care plan drafting, patient communication, research, and reporting - and to actively shape how Laina supports pelvic floor patients between visits.
How You'll Spend Your Time
Clinical Care & Caseload
- Evaluate and treat pelvic floor patients virtually - history, external observation, movement and breath assessment, patient-guided self-assessment, and evidence-based exercise and behavioral intervention.
- Carry a caseload. Set engagement, adherence, and outcomes targets in collaboration with leadership and take ownership of delivering against them.
- Treat general orthopedic and musculoskeletal patients, who will make up much of your caseload early on and become secondary as pelvic referrals grow, applying the same standard of care and documentation
- Identify when a patient needs in-person pelvic floor care, imaging, or physician follow-up, and route them appropriately within the partner's network.
Program Build & Clinical Design
- Define the pelvic floor care model: episode length, visit cadence, exercise progressions, home program structure, and how Laina supports patients between visits.
- Build the pelvic floor exercise library and condition-specific protocols - adding new exercises, cues, and modifications as needed - and work with product to get them into the platform.
- Select and implement outcome measures by condition, set the collection cadence, and establish how results are reported back to partners.
- Map the program to billing: CPT code selection, documentation requirements, and payer-specific considerations for virtual pelvic floor care across states.
Program Growth & Leadership
- Serve as the clinical face of the program to partners - joining partner calls, answering clinical questions, and translating partner needs into program changes.
- Codify what's working - intake criteria, protocols, red flags, escalation paths - into a repeatable playbook so future pelvic floor hires can be onboarded quickly and consistently.
- Bring a structured point of view to leadership on which conditions to deepen, which to add next, and where virtual care is not the right fit.
- Partner with LainaHealth's clinical, product, and operations teams to translate patient and partner signal into product priorities.
Qualifications
- Required
- Doctor of Physical Therapy (DPT) from a CAPTE-accredited program.
- Active, unrestricted physical therapy licensure in multiple U.S. states, or a PT Compact privilege with a home-state license, and willingness to obtain additional state licenses as program coverage expands.
- 3+ years of clinical experience treating pelvic floor and pelvic health conditions, with a caseload that has included pregnancy and postpartum, urinary and bowel dysfunction, pelvic pain, and prolapse.
- Direct experience treating pelvic floor patients virtually - evaluating, treating, and progressing patients through telehealth without hands-on or internal assessment - and a clear point of view on what works remotely and what does not.
- Strong orthopedic and musculoskeletal clinical skills, and willingness to carry a substantial orthopedic caseload while the pelvic program ramps
- Demonstrated ability to operate in ambiguous, early-stage environments: building protocols from scratch, making clinical decisions without a playbook, and iterating based on patient and partner feedback.
- Startup mentality: resourcefulness, adaptability, and a bias for action over process.
- AI-integrated workflow. Must actively and intentionally use AI tools across day-to-day work - documentation, care plan drafting, patient communication, research, and reporting - and be able to articulate how AI is woven into your clinical process.
- Preferred
- Pelvic health specialty credentialing - APTA Academy of Pelvic Health CAPP-Pelvic or CAPP-OB, ABPTS Women's Health Clinical Specialist (WCS), or Herman & Wallace Pelvic Rehabilitation Practitioner Certification (PRPC).
- Experience building or leading a pelvic health program, service line, or clinic - including hiring, training, or mentoring other pelvic floor clinicians.
- Experience with patient-reported outcome measures in pelvic health and how outcomes data is used by health systems and payers.
- Experience in a digital health or virtual-first practice, including familiarity with telehealth documentation, consent, and cross-state practice requirements.
- Existing PT Compact privileges in a majority of Compact member states.
- Who This Isn't For
- This role is likely not the right fit for:
- Clinicians who want a fully built program with established protocols, a trained team, and a set caseload - there is no program yet, this role builds it.
- Pelvic health specialists who only want to treat pelvic floor - orthopedic and musculoskeletal patients are part of the job, especially early on.
- Clinicians who believe pelvic floor care cannot be delivered well without hands-on or internal assessment - this role requires conviction that virtual pelvic floor care works, and the experience to back it up.
- Operators who need heavy process and predictable playbooks before taking action - this role invents the playbook in parallel with treating.
